Titles are obtained by doing various things in-game. There are hundreds of titles and not all of them have been discovered yet. You can change your title by opening the character window and clicking change by the title.

Monster Joke Punchline - Die 10 times in a row and lose 10% exp from the same monster (didnt work for me does being on a pet make a difference?)

Friend Seeker - Have 5 friends

Friendly - Have 10 friends

Popular - Have 30 friends

Celebrity - Have 50 friends

Announcer - Shout 100 times in 1 hour

Broadcaster - Shout 500 times in 1 hour

Talk Show Host - ?

World DJ - ?

Apprentice Enhancer - Successfully enhance 3 items in a row

Rookie Enhancer - Successfully enhance 5 items in a row

Skillful Enhancer - Successfully enhance 6 items in a row

Triumphant Enhancer - Successfully enhance 10 items in a row

Miraculous Enhancer - Successfully enhance 15 items in a row

Lucky Enhancer - Successfully enhance one item to +3

Successful Enhancer - Successfully enhance one item to +6

Enhancement Artist - Successfully enhance one item to +8

Unlucky Enhancer - Fail to enhance 5 items in a row

Failed Enhancer - Failed to enhance 20 items in a row

Hopeless Enhancer - Failed to enhance 30 items in a row

Cursed Enhancer - Failed to enhance 50 items in a row

Item Spoiler - Destroy 5 items in a row during enhancing

Item Breaker - Destroy 20 items in a row during enhancing

Item Destroyer - Destroy 50 items in a row during enhancing

Item Bankrupt - Destroy 70 items in a row during enhancing
