A Mushroom may drop either a toadstool or a mushroom.

Mushroom (Low Quality) - Note: Sea of Greed Squares H4-H5, Forest of Mist
Mushroom (Normal Quality) - Note: Sea of Greed Squares H4-H5, Moonlight Tomb, Sand Hill
Mushroom (High Quality) - Note: Ancient Elven Woods Squares F5-F8
Mushroom (Highest Quality) - Note: Ancient Elven Woods Squares F5-F8

Toadstool (Low Quality) - Sea of Greed H4-H5, Forest of Mist
Toadstool (Normal Quality) - Sea of Greed H4-H5, Moonlight Tomb, Sand Hill, Vine Tomb
Toadstool (High Quality) - Uruga, Ancient Elven Woods Squares F5-F8
Toadstool (Highest Quality) - Ancient Elven Woods Squares F5-F8

Sea Mushroom is an early quest item and can be found in the Sea of Greed in square H5.
