I played it for a few people who had trouble with it. It was pretty fun, comparable rush to when you're barely managing to keep people alive. Then again, I was a virgin non-CS user on my original play-through so had to use all +0.
If they haven't changed anything since I played, it's pretty much like the previous posters said. Gaining aggro purely through heal-spam seems to be less effective than in normal-maps so cycle Bashes on mobs that are attacking the NPCs.
I don't know if it's true, but I've heard that Roumen has more HP/Def than Elderine so they said to prioritize getting mobs off of her. Take that with a grain of salt.
They spawn in clumps so just wade in there and gain aggro.

like the other said, healing to get aggro isn't as good as just tabbing through the mob and hitting each until you get aggro. That is what I did, and it works best.

it like Janesama said,its not as hard as everyone says. i successed on my first try on my cleric. just make sure the mobs attack you most of the time and you`ll be fine. but i had to spam every heal i had
Restore doesn't gain aggro. You'll want to be spamming the crud out of Rejuvenate. That is a major aggro stealer. Also don't camp at Eld/Roum, go out and meet the mass from one of them. This is a good way to keep them off the heroes, and also generally the heroes will help each other this way, just make sure to watch them.
Don't use a def/eva scroll you want them hitting you and hitting you harder so you can heal more and pull more agro. Use a vit and aim if you use a hammer. Also if it helps keep them on you leave off your pants and boots. Keep the top for the HP boost assuming you use a 55 top. This will allow you to heal for more and keep them on you. Towards the end you'll need to put them back on probably though for the defense.
