I have a sneaking suspicion it's all fake. Either that or those people have never had a conflict at a resturant before. "May I see a manager, please?" is the best way to go once things start getting weird with a server. The minute she brought back a plate with the trash still on it I would have jumped above her head. Calmly, mind, but you just don't deal with that level of nonsense.
