Constant Warping Around in Vine Tomb

I don't know what's going on but for some reason my character keeps disappearing and reappearing in various unaccessible areas in Vine Tomb. I don't know why it's doing this but does anyone have an idea? Here's an example:

Does it happen everytime or every so often?

Every so often, like for example I rest awhile at a spot. It lags a bit and I reappear somewhere else. Can't figure out why though..

Somehow I ended up in Roumen after I relogged. It's like I found a secret tunnel...
