Reasons for failure that i've seen so far:
Slime KQ: Too many lower levels, too many clerics that attack and don't heal, people that don't know wtf they're doing.
Mara KQ: People pulling fakes, people trying to run past all the mobs instead of fighting them because they think they'll get a better prize, "ZOMGZ DUN STUN MARA SHE RESET AGGRO!1!!1!1!!!" so Mara ends up beating down on them when they could've just stunned her and been fine, and fighters that can't keep aggro on mobs/bosses and everyone else ends up getting owned because of it.
Gold Hill: "LOL I GOTZ KEY *Shops it for silver*" or "LOL I GOTZ KEY EVER1 GIVE ME 2s N I'LL OPEN GAET.", not enough people mining so we don't find the key in time, or people opening the boss gate before anyone has time to rest so the boss tromps around and owns us all.
Robo KQ: People that don't know what to do, bad runners, no clerics (this is a KQ where you seriously need them imo, the others I think are passable without them.), too many no-healing clerics, fighters not keeping aggro when they need to, too many lower-level people.
