The being labelled as ???'s in pvp is a good idea. But maybe instead of being named ??? being named your current placing. Itd be helpful to figure out who your up against, and to prevent teaming.

Seems logical to me. I mean once you have a bunch of scrolls, pots, quest items, perm items taking up your inventory, you can only have a hand full of spots to collect mats.

A good friend of mine claims to have gotten them once from a hero mob (mutant wolf, king boogy... huge greenky etc) but ive never had success with that, i just always get a named weapon from them but...
