The secret is life mines and self-casting Rock+Life Element or any other Life AoE cause beams can miss when he does the circle-thing. Use the AoE when the real-one appears and hes in position to slash. Last-second TP works too if you want to be badass...?
Seed the area with life-mines in the beginning of the fight to blow up any skeletons he summons. When he starts coming at you incorporal-form to slash, teleport so that you're right next to life mines and that they're in-between him and you. When he comes to slash you, he turns corporal and gets massively damaged when he trips the mines.
More damaged if he gets every mine in the face, even more if you charge 4 life and 1 shield into the mines.
Edit: Haste is vitaaaaaaaalllll. Can outrun death's most all of the time.
