I have heard many ways to get this title, but nothing seems to be concrete. What I do know is that I was with a lvl 15 cleric in CP when he received the title. I did not count how many times he died... but it was certainly many. However he did not seem to get the title until after a left him on a ridge and he died so a revived him and then very soon after was dead again and received the title at that time. However, I was with a fighter once watched him die 10+ times and with me reviving him, but he never received a title. I had a theory that maybe you just need to be killed by a really high lvl monster, so I had Legendary Tree kill me a number of times, but no title there.

Exp loss is not a requirment! Cpt_Amazing just got the title hunting GGK by dying to mages a lot. He had the Blessing of Teva so he lose no exp.

Does it count if a cleric revives you?

I may get Loki to help me out and try this thing out. It takes a King coll about 4 hits to take me out, and if Loki can rez me ten times, then I should be able to get it within 10 minutes tonight.

well we tried to get MJP for blah's mule in CP...

she got killed by harkens like 20 times and i revived her all 20 times.. and she never got the title... (her mule was about lvl 20 i think)

i wonder if u have to wait a bit between kills.. coz we did it very quickly

Hmmm.... not good. I wonder what is the actual steps you have to take inorder to get it?

I'm pretty sure it something to do with getting 1-hitted a few times. I had mine at lvl 5.
